Three Chicago nursing homes hit with health violations

Three nursing homes in Chicago have been cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the second quarter of 2017.

The report, released on Monday, lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively.

Harmony Nursing & Rehab Center, a 180-bed skilled care facility at 3919 W. Foster Ave., Chicago, received a citation for an “A” violation and a $25,000 fine for failing to conduct a follow-up assessment of an eye condition after a resident was admitted to the hospital with a conjunctival infection, and conduct a skin assessment in which deep tissue injuries were found. The facility has requested a hearing.


Wentworth Rehab and HCC received two “A” violation citations and a $50,000 fine for failure to provide emergency care to a resident involved in a fire, and to report to the physician the inability to follow a resident’s care plan. The facility, a 300-bed skilled care facility located at 201 W. 69th St., Chicago, has requested a hearing.

Mado Healthcare – Douglas Park, a 172-bed intermediate care facility located at 1550 South Albany, Chicago, has been cited with an “AA” violation and fined $50,000, stemming from an incident where they allegedly failed to intervene when one resident physically assaulted another, resulting in hospitalization and subsequent death of the resident. The facility has requested a hearing.
