The Illinois Department of Commerce and Economic Opportunity is seeking input from businesses that are affected by COVID-19. This survey will assist in evaluating a request for an economic injury disaster declaration from the U.S. Small Business Administration. That declaration will allow for businesses to apply for low-interest loans to keep their businesses open during this challenging time.
